Sample RFP for Chapter Subawardees

Dear Chapters,

Please see below the sample Subawardee RFP as well as the Subawardee application forms. Please note that this is a hidden page that is only available to Chapter grantees. Please do NOT disseminate these forms until your Subawardee RFP is approved by NCA.  Proposed changes need to be in accordance with the NCA’s  RFP FY2009, the grantee guidelines, the signed Cooperative Agreement and the originally approved Chapter application.

In order to submit your Subawardee RFP to NCA:

• Go to https://www.grantrequest.com/SID_1093/?SA=AM and log into your grant application account. You need to use the same log-in that you used when you submitted your application. Click on the “Requirements” link at the top left corner.
• Under “Form Name,” go to “Subawardee RFP” link.  Scroll down the page “Grant Information” and hit next.
• Please upload your document (in a .doc format) using the “Browse” and “Upload” buttons. Scroll down the page to “Review and Submit” and go to the next page. After reviewing your submission please go down the page and hit “Submit” again. After a successful submission you will receive a confirmation email. Please keep it for your files.

**Deadline for the subawardees to submit their applications to their Chapter is no later than  April 1, 2010.

**Deadline for submitting the reviewed  subawardee applications and grant budget to NCA is no later than April 15, 2010.
